    Started taking it in 2004, fur injuries received woke deoloyed. Got out of the army, remained on methadone. After one too many times with the VA being late delivering my meds, I sought out an outside doc. Va doc cut me off after finding outside doc. I've only taken methadone. Nothing elsr. No stree drugs. Va got me outpatient sub clinic...a month from now. ..but im not able to be sick and drop out of nursing school, and..have two little girls whom depend on me. I begged the VA doc for just enough methadone ie 30 mg..to keep me noreal until outpatient treatment program. No dice. Hate the va. Anyway...found a cash addiction specialist psych whI'm I know prescribes sub...so I'm going to him at 1600. My fear is....I really want to do this right...as comfortable as I can while making the transition. Figure I'll end up on sub but want to get to 30 mg methadone fur a while before...as per guidance here. I hope he will help me, particularly because his bill is 250 bucks first time. I'm scared. What is the right way to do this? 70 mg methadone for 11 years. Sometimes more if there were overlap between doc and va but always 70.

    Welcome to the forum. There IS a right way to make the transition from methadone to subs. Theres a wrong way also. The right way is to get the methadone dose as LOW as possible, but at least to 30mgs as you mentioned finding out here. Then it's HIGHLY recommended using the COWS to safely make the sub induction.

    COWS = Clinical Opiate Withdrawal Scale

    Please don't make a guessing game out of it by figuring enough "time" has elapsed between the last dose of methadone and being able to take the first dose of subs. Many have made that mistake and went into PWD's, which I can verify totally SUX, going into them TWICE because I got in a hurry making my own switch to subs from methadone.

    You'll need to get to a score of 26 or higher before induction. And because of the long half life of the methadone it could take DAYS! Did for me. I was down eventually to 10mgs of methadone and it still took nearly 90 hours before I got to that 26 score. I made the mistake of NOT using the Cows the first 2 tries and paid an awful price. Some may say 24 hours is ok, maybe 36 hours will work. WHY take the chance, use that COWS!!!

    After taking your last dose of methadone, and hopefully it's low, you'll have to wait until you're in moderate to severe withdrawals and that is definitely no fun whatsoever. But it's positively necessary. Take NOTHING that makes you feel better during that time. The entire point is to be sick with symptoms so that Cows score gets higher. If you take any kind of comfort med it could delay that process and have you suffering longer than necessary. Not what you want I assure you.

    That's the RIGHT way to make your switch to subs. I will be happy to help all I can if needed. Continue to post often. It helps. Take care.

    Been off methadone. Took sub at 30 hour. Feel fine...only side effect is a terrific headache. Outside of that....I'm good. But these headaxhes. Ouch. Had tapered down to 30mg before making the jump. That and the gradual Low inducing. We're a godsend. No paws. Just headache. Tapered to 30 prior to the jump. Just headache. Low gradual induction. Doc prescribed 2 8mg a day. But I didn't take but 6 total.

    Stupid smartphone syntax. I'm really not schizophrenic. but yes.. headache. That and ravenous appetite.

    72 hours off methadone. Feel ok...mild anxiety and headache. But I'll take it. The first thing I notice is that I don't feel sloooo ow like I did on mdone...I don't lack ambition

    feel even better today. so, I seem to be a case that lies in the margins, the outlier...wrt PWS. I had very mild symptoms, even after being on methadone for 11 years with no break. I dont have the proper perspective on this thing right now, but after a week , ill see. but right now...feel fine. even, dare say. Good.
