    Hey guys, I've finally registered after lurking threads on these forums for years throughout my research and personal curiosity. I believe that I have some information to share that could potentially be helpful to some individuals, and felt it was my turn to give back to a community that has provided me with a lot of helpful information throughout the years.

    I have personally tapered, and have helped many other individuals taper off of a wide array of opiates. Starting at the age of 12 I've struggled on and off with addiction to an abundance of different kinds of opiates (prescription and illicit). Through my research I have found several different tapering guides, however I have tried many of these and have always struggled with some kind of withdrawal, getting worse towards the lower doses.


    In my experience, myself and the individuals that have used this approach have had a great success with tapering off of opiates. This approach is highly modifiable and can be as gradual or abrupt as you chose for it to be. Below I will explain the approach that has worked for me and several others taper off of Suboxone, Oxymorphone, Oxycodone, Hydrocodone, and even "Diacetylmorphine".

    For simplification purposes and clarity I will be using an imaginary dose of 100mgs daily of an imaginary opiate. Let's say you are currently in the habit of dosing yourself about 4 times daily in 25mg intervals (most of the people I have helped usually dose between twice and four times daily, so I will start with four doses however this can be modified to work with any dosages and time between doses). Our first goal will be to get you to getting accustomed to dosing yourself once a day, before we even begin tapering. This will work better and be easier to do with opiates of a longer halflife such as Suboxone and Methadone, but it can be done with shorter acting opiates as well, and I highly encourage this approach if you are struggling with other tapering approaches.

    Ok so we're starting at four doses of 25mgs for a total of 100mgs of said substance a day (as an easy visual, the "mgs" can be thought of as percentages and applied to any dose of any opiate - ex: 25mg = 25%). Lets get it to 1 100mg dose before tapering at all. Let's say you wake up at 8:30 every morning and dose yourself four times a day, your dosing might look like this on a typical day: 25mg (9:00-10:00am), 25mg(1:00-3:00pm), 25mg(5:00-7:00pm), 25mg(9:00-11:00pm) Total - 100mgs.

    Obviously I do not advise you immediately begin taking 100% of your total daily dose of opiates all in one single dose, that could be potentially very dangerous, but over time that will be our basic goal (however, 9/10 times this process has been able to not only reduce the number of doses per day, but also the total daily intake - which will only help you to reach your end goal more quickly, which is why I recommend this step first). Here's how I've been able to do that:

    We will be doing transitions in 4 day cycles as that's the maximum average half-life of Suboxone, a long lasting opiate. If you are using a shorter acting opiate, you should still use this 4 day cycle if you are struggling with a different taper, as it will only make your experience that much more gradual, and easier

    MG = % (For easy modification to any dosages)

    FIRST STEP:

    Day 1: 25mg (9:00-10:00am), 25mg(1:00-3:00pm), 25mg(5:00-7:00pm), 25mg(9:00-11:00pm) Total - 100mgs
    Day 2: ~33mg(10:00am-12:00pm), ~33mg(4:00-6:00pm), ~33mg (9:00-11:00pm) Total - 100 mgs
    Day 3: 25mg (9:00-10:00am), 25mg(1:00-3:00pm), 25mg(5:00-7:00pm), 25mg(9:00-11:00pm) Total - 100mgs
    Day 4: ~33mg(10:00am-12:00pm), ~33mg(4:00-6:00pm), ~33mg (9:00-11:00pm) Total - 100 mgs

    Day 1: 25mg (9:00-10:00am), 25mg(1:00-3:00pm), 25mg(5:00-7:00pm), 25mg(9:00-11:00pm) Total - 100mgs

    So now we have went from four 25mg doses daily to three ~33mg doses daily (I say "~33mg" because many people find it easy in this stage to start slowly chipping away at their total daily intake in this step, so if you can take less than 33% of your dose, even if you're only taking less during one dose per day, every mg you can cut out will only hasten your detox process) I will give an example of this modified version of the above approach, for those who would like to speed this process up a bit:

    FIRST STEP (MODIFIED):

    Day 1: 25mg (9:00-10:00am), 25mg(1:00-3:00pm), 25mg(5:00-7:00pm), 25mg(9:00-11:00pm) Total - 100mgs
    Day 2: ~33mg(10:00am-12:00pm), ~30mg(4:00-6:00pm), ~33mg (9:00-11:00pm) Total - ~96 mgs
    Day 3: 25mg (9:00-10:00am), 25mg(1:00-3:00pm), 25mg(5:00-7:00pm), 25mg(9:00-11:00pm) Total - 100mgs
    Day 4: ~33mg(10:00am-12:00pm), ~30mg(4:00-6:00pm), ~33mg (9:00-11:00pm) Total - ~96 mgs

    This modified version will help you lower your total daily intake as your body adjusts to the new dosing schedule, you are less likely to notice withdrawals from very slight changes in doses.

    Now that we have hopefully gotten down to 1 daily dose we can begin the actual tapering. I'm going to reset the doses (percentages) back to be on a 100mg (%) daily dose scale to provide a more clear visual of this process:

    Continue as long as needed, let's go to 1mg for a good example of a continued Suboxone taper (or other opiates if you're still experiencing anxiety when it comes to skipping doses)

    I find it extremely difficult to split the tiny .125 slivers into .063 dots... but it can be done and anyone who doesn't wish to skip a day will find a way to do it I'm sure, however anyone who feels comfortable skipping days can do so at this point with little to no noticeable w/ds

    For those of you that were able to skip days and taper completely off, congratulations!! You did it!
    For those that continued down to .063 dots per day, use the above plan to taper off completely (just replace the .125mg doses with .063mg, and the .063mg doses will be your skip days). Honestly at this dose, the drug is having little to no effect on you anymore and you could probably jump off now and be fine, but take your time and go at your own pace and you will have the highest personal success rate with completing the taper, and have the smallest risk of relapse.

    I apologize for this extremely long post, but some people will appreciate the added detail and if I can even help one person successfully taper off of their current opiate habit, then this post was successful in my eyes. I will check back to answer any questions posted in the thread, and will be more than happy to answer them, or elaborate on any information I've already posted.

    You obviously have a lot of experience, and I appreciate your advice, but I think you should revise your post with less numbers and percentages. Mainly because even though your using 100mg as an imaginary amount, you are still advising someone to eventually take their total daily amount in one dose before tapering. If I read this post two months ago, I was take 600-800 mg of oxycodone. If I couldn't get those, I would take about 500mg of hydrocodone for the day. So had I followed your method, I would of taken all that In one dose and prob overdosed. Nothing personal, because you do know what your talking about... But I think you jumped the gun a little before considering the potential harm it could cause. Please read my taper method and let me know what you think.
